    Description

    Avonza is composed of three active compounds, including Lamivudine (3TC) and Tenofovir (TDF), which are nucleotide reverse transcriptase inhibitors (NRTIs), and Efavirenz (EFV) – non-nucleoside reverse transcriptase inhibitor (NNRTI).

    Indications:

    - human immunodeficiency virus type 1 (HIV-1) infection in adults 18 years of age and older.

    MOA (mechanism of action):

    Tenofovir and Lamivudine are known as NRTIs and they work by interfering with the normal working of an enzyme reverse transcriptase that is crucial for the HIV to reproduce itself. Efavirenz binds directly to the HIV-1 RT, an RNA-dependent DNA polymerase, suppressing its function in viral DNA replication. Therefore, Avonza reduces the viral load in blood, increases the count of CD4 (T) cells in blood which aids in warding off other infections. Tenofovir, Lamivudine, Efavirenz are not active against HIV-2.

    Dosage

    The usual dose of Avonza is one tablet once a day, at the same time.

    Take Avonza on an empty stomach (2 hours after food) at bedtime. Bedtime dosing helps reduce the impact of some of the side effects, especially during the first few weeks.

    Do not run out of Avonza. Refill the prescription or talk to your doctor before Avonza is gone.

    Precautions:

    HIV can be transmitted through sexual contact with infected partner, blood (e.g., sharing needles, handling blood instruments, receiving blood transfusions, blood products, or organ/tissue transplants that were infected with HIV), childbirth, toiletries (razors and toothbrushes). Therefore, both infected and non-infected individuals are advised to have any kind of sex using a latex or polyurethane condom, avoid sharing needles or other things like toothbrushes or razors.

    It is important to discuss with doctor the precautions needed to avoid infecting other people as well as take precautions not to get infected.

    Women who are HIV-positive must not breastfeed because HIV infection can be passed on to the baby in breast milk.

    Some people who take Avonza may experience:

    - Lactic acidosis (a buildup of lactic acid in the blood). The symptoms are unexpected tiredness or weakness, nausea or vomiting, constant abdominal pain, pancreatitis. If any of these symptoms occur, seek emergency medical attention immediately.

    - Severe acute exacerbations of hepatitis B may develop in patients who are co-infected with HBV and HIV-1 and have discontinued tenofovir disoproxil fumarate or lamivudine, both of which are components of Avonza. Hepatic function should be monitored closely with both clinical and laboratory follow-up for at least several months in patients who discontinue anti-hepatitis B therapy including Avonza and are co-infected with HIV-1 and HBV. If appropriate, initiation of anti-hepatitis B therapy may be warranted.

    - Immune Reconstitution Syndrome – a condition when immune system may get stronger and starts fighting infections that have been hidden in the body for a long time.

    Missing a dose

    If you notice a missing within 12 hours of the time you usually take Avonza, you must take the tablet as soon as possible. Then take the next dose as usual. If you notice 12 hours or more after the time you usually take Avonza, then do not take the missed dose. Wait and take the next dose at your usual time. Do not take a double dose of Avonza to compensate the missed dose.

    Overdose

    If an overdose occurs and you are not feeling well, stop taking the medication immediately. You should seek emergency medical attention or contact your healthcare provider immediately.

    Storage

    Store Avonza tablets or oral powder at room temperature between 68-77 degrees F (20-25 degrees C). Keep Avonza in the original container. Do not use Avonza if the seal over the bottle opening is broken or missing. Keep the bottle tightly closed. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

    Side effects

    The most common and uncommon side effects associated with Avonza are:

    Side effects occurrence does not only depend on the medication you are taking, but also on your overall health and other factors.

    Contraindications

    Avonza is contraindicated in patients who are allergic to Lamivudine, Tenofovir, Efavirenz or any of the other ingredients of Avonza.

    Do not use Avonza in patients under 18 years of age and in patients over 65 years of age.

    Do not let your Avonza run out. Refill your tablets if therapy is continued.

    Do not stop taking Avonza suddenly and without first talking to the doctor.

    Avonza is not indicated for the treatment of chronic hepatitis B (HBV). Thus, Avonza isn't for use in patients co-infected with HBV and the human immunodeficiency virus type 1 (HIV-1).

    Do not take Avonza if you have liver problems, including HBV infection, kidney problems, bone problems, a history of mental illness (depression, alcohol abuse), a history of convulsions (fits or seizures), diabetes, high cholesterol.

    Do not combine Avonza with other medicines that contain Tenofovir (Complera®, Stribildtm, Truvada), adefovir (Hepsera®), didanosine (Videx, Videx EC), atazanavir (Reyataz), lopinavir with ritonavir (Kaletra), darunavir (Prezista), ledipasvir with sofosbuvir (Harvoni®), sofosbuvir with velpatasvir (Epclusa®), astemizole or terfenadine, bepridil, cisapride, ergot alkaloids, midazolam or triazolam, pimozide, voriconazole, acyclovir, adefovir dipivoxil, cidofovir, ganciclovir, valacyclovir, valganciclovir, aminoglycosides (e.g., gentamicin), anticonvulsants (carbamazepine, phenytoin, phenobarbital), immunosuppressants, anticoagulants, antidepressants (Sertraline), antifungals (itraconazole or posaconazole), methadone (to treat opiate addiction), high-dose or multiple NSAIDs, products containing St John’s wort, calcium or iron supplements, multivitamins.

    Women should not become pregnant and should use two effective birth control methods while taking Avonza and for 12 weeks after stopping it. A barrier form of contraception like condom, contraceptive sponge, diaphragm, cervical cap should always be used along with another method of contraception.

    Hormonal contraceptives like pills, injections, vaginal rings, or implants may not work during treatment with Avonza.

    Do not take Avonza if you are pregnant or plan to have baby, are breastfeeding or plan to breastfeed.

    Women who are HIV-positive must not breastfeed because HIV infection can be passed on to the baby in breast milk.

    Alcoholism, hepatic disease, hepatotoxicity or lactic acidosis, obesity, organ transplantation are contraindications for using Avonza.

    Do not have any kind of sex without using a latex or polyurethane condom.

    Do not share needles or other things like toothbrushes or razors.
