    Pilocar® ophthalmic solution is the drug of choice for emergency lowering of intraocular pressure (IOP) of both open-angle and angle-closure glaucoma. These drops can also be used for prevention of IOP associated with laser surgery. Pilocar® eye drops cannot be used to treat pupillary block glaucoma, eye inflammation, or a severe eye infection.

    Description

    Pilocar® solution is manufactured by FDC. The active constituent in Pilocar® eye drops is Pilocarpine, a nonselective muscarinic agonist used to treat xerostomia and glaucoma.

    Indications:

    - increased intraocular pressure(IOP) in patients with glaucoma;

    - angle-closure and angle-closure glaucoma;

    - prevention of IOP associated with laser surgery.

    Pilocar® eye drops can be used for the treatment of xerostomia, that is characterized by dry mouth and lack of tears. But the use of Pilocarpine for producing these effects has been limited due to its lack of selectivity.

    Pilocar® eye drops are also useful in reversing mydriasis due to atropine (anti-muscarinic agent resulting in dilation of the pupil).

    MOA (mechanism of action):

    Pilocarpine is an ophthalmic glaucoma agent, used to reduce eye pressure in glaucoma. Applied topically to the eye, Pilocarpine produces rapid miosis and contraction of the ciliary muscle. When the eye undergoes this miosis, it experiences a spasm of accommodation. The vision becomes fixed at some particular distance, making it impossible to focus.

    Dosage

    For increased intraocular pressure (IOP) in patients with glaucoma

    Adults and children over 2 years: instill 1 drop into the treated eye(s) up to 4 times per day.

    Infants under 2 years: instill 1 drop of the 1% ophthalmic solution into treated eye(s) 3 times daily.

    For treatment of acute angle-closure glaucoma

    Adults and children over 2 years: initially instill 1 drop of 1% or 2% Pilocarpine solution into treated eye(s) up to 3 times over a 30 minute period.

    After laser iridoplasty, instill 1 drop of 1% Pilocarpine solution 4 times daily until an iridotomy can be performed.

    Infants under 2 years: instill 1 drop of the 1% Pilocarpine ophthalmic solution into treated eye(s) 3 times daily.

    For the prevention of IOP associated with laser surgery in adults only

    Instill 1 drop (or 2 drops 5 minutes apart) of 1%, 2%, or 4% ophthalmic solution into the treated eye(s) 15 to 60 minutes prior to surgery.

    For miosis (contraction of the pupil of the eye) induction

    Adults: instill 1 drop (or 2 drops 5 minutes apart) of 1%, 2%, or 4% ophthalmic solution into the eye(s).

    Infants and children: for the induction of miosis prior to goniotomy or trabeculectomy, instill 1 drop of 1% or 2% ophthalmic solution into the eye 15 to 60 minutes prior to surgery.

    Precautions

    Pilocarpine-naive patients should start with the lowest available concentration of Pilocarpine solution as higher concentrations are often not tolerated initially.

    Pilocar® solution for topical ophthalmic use only, not for injection.

    Pilocar® solution is associated with allergic reactions, even following a single dose. Discontinue using Pilocar® immediately and contact your physician at the first sign of a rash or allergic reaction.

    Pilocar® drops may cause blurred vision. Do not drive or operate machinery while using the drops.

    Avoid contaminating the applicator tip with eye, fingers or another source.

    Patients should not wear contact lenses while using Pilocar® eye drops.

    Remove contact lenses before using Pilocar® eye drops.

    Wash your hands before using the eye drops.

    Missing a dose

    If you miss a dose you should instill it as soon as you remember about your missing. If it is the time for the next dose you should continue your regular dosing schedule. Do not instill a double dose to make up for a missed one.

    Overdose

    If an overdose occurs and you are not feeling well, stop taking the medication immediately. You should seek emergency medical attention or contact your healthcare provider immediately.

    Storage

    Store at a cool temperature, away from excess moisture and heat. Do not freeze. Keep out of the reach of children.

    The most common side effects associated with Pilocar® solution are:

    • allergic reactions
    • blurred vision
    • change in vision
    • eyelid twitching
    • headache at the temples or around the eyes
    • increased tearing
    • nearsightedness
    • redness or swelling of the eye
    • temporary stinging or burning

    Side effects occurrence does not only depend on the medication you are taking, but also on your overall health and other factors.

    Contraindications

    Pilocar® solution is not for injection.

    Do not use Pilocar® eye drops if you are allergic to Pilocarpine.

    Do not use Pilocar® eye drops if you are allergic to any ingredient comprised in Pilocar® solution.

    Do not use Pilocar® eye drops if you have a certain type of glaucoma (e.g., pupillary block glaucoma), eye inflammation, eye infection, a detached retina, or chronic obstructive pulmonary disease.

    Do not use Pilocar® eye drops if you have if you have had a heart attack.

    Discontinue using Pilocar® eye drops immediately and contact your physician at the first sign of a rash or allergic reaction.

    Do not to use Pilocar® eye drops if you are pregnant, plan to have a baby or are breastfeeding.
