    Description

    Budecort® Rotacaps are manufactured by Cipla and contain the active ingredient Budesonide. Budesonideis a glucocorticoid that has the ability to inhibit the chemicals in the body which promote inflammation in the lungs. This medicine goes directly into the airways and lungs, producing its effect. It works by scavenging harmful mediators of inflammation and preventing the formation of excess mucus, which causes the obstruction of the airways. This way it alleviates asthma symptoms and facilitates smooth breathing. Budecort® Rotacaps are inhaled in the form of dry powder and are to be used on a long-term basis. This drug doesn’t produce immediate relief. In case of an acute asthma attack, a short-acting medicine is required to be used.

    Indications:

    - patients requiring oral corticosteroid therapy for asthma;

    - maintenance treatment of mild asthma in patients requiring an intermittent symptomatic bronchodilator;

    - treatment of moderate asthma in patients requiring regular asthma maintenance medicine;

    - patients with severe chronic asthma.

    MOA (mechanism of action):

    Budecort® Rotacaps with the active ingredient Budesonide keep check on the rate of Protein synthesis and lowers the movement of Polymorphonuclear Leukocytes, Fibroblasts. The drug has a high topical glucocorticosteroid activity that promotes the body’s cells to decrease the release of certain chemicals (mast cells, eosinophils, neutrophils, macrophages, and lymphocytes, histamine, eicosanoids, leukotrienes, and cytokines) which are involved in the immune system, and which initiate an inflammatory reaction. Thus it reduces inflammation. In asthma, the airways tighten due to inflammation and can also be blocked by mucus. This makes it difficult for air to get into and out of the lungs. By preventing the inflammation and excess mucus formation, Budesonide helps prevent asthma.

    Dosage

    The dosage highly depends on the severity of asthma and to be prescribed by a doctor for each patient individually. Budecort® Rotacaps should only be inhaled with the help of the Cipla Rotahaler/ Revolizer device. The medicine must not be swallowed.

    The recommended dosage for the asthma therapy is following:

    Adults:

    - 400 mcg/day in 2 divided doses;

    - maximum dose is 1600 mcg/day.

    Children (5 – 12 years):

    - 200-400 mcg/day in two divided doses;

    - maximum dose is 800 mcg/day.

    Asthmatic patients should always be supervised by a specialist and take medical tests on a regular basis to check their asthma conditions.

    How to use your Budecort® Rotacaps with the Rotahaler device:

    1. Take one of the capsules from the packaging and insert into the Rotahaler.

    2. Twist the Rotahaler to pierce open the capsule.

    3. After the capsule is pierced, take a complete dose of the medicine by breathing in by mouth two times.

    4. Throw away any capsules which are not used right away after it is taken out of the package. Do not leave the capsules open to air, they may not work as well.

    How to use your Budecort® Rotacaps with the Cipla Revolizer device:

    1. To open your Revolizer, hold it at the base with one hand and pull back the mouthpiece.

    2. Insert the rotacap into the rotacap chamber with the transparent end facing down.

    3. Close the mouthpiece firmly. A 'click' sound will indicate a proper closing of the Revolizer.

    4. Breathe out fully, through the mouth.

    5. Place the mouthpiece between the teeth and close the lips tightly around it.

    6. Keep your head straight and breathe in through the mouth rapidly and deeply.

    7. Remove the Revolizer from your mouth and hold your breath for about 10 seconds or for as long as it is comfortable and then breathe out normally.

    8. In case some powder remains in the rotacap chamber, repeat step 4. Ensure that no powder remains in the rotacap chamber.

    9. Open the mouthpiece after every use and discard the empty rotacap.

    How to clean your Revolizer device:

    1. Open the mouthpiece of the Revolizer.

    2. If there is an empty rotacap in the chamber, remove it.

    3. Rinse the mouthpiece and rotacap chamber in clean running water.

    4. Shake well to remove excess water and leave it to air dry.

    5. Clean the Revolizer at least once a week.

    6. Never use the Revolizer when it is wet.

    Missing a dose

    If you miss a dose of Budecort® Rotacaps you should take it as soon as you remember. If it is the time for the next dose you should continue your regular dosing schedule. Do not exceed the doctor’s recommended dose.

    Overdose

    If an overdose occurs and you are feeling unwell, you should seek emergency medical attention or contact your healthcare provider immediately.

    Storage

    Store at room temperature between 15 and 30 degrees C (59 and 86 degrees F) and away from excess moisture and heat. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

    The preparation is not recommended in patients with liver injury. The dose should be reduced based on the clinical condition.

    Do not use Budecort® Rotacaps in patients younger than 5 years of age without the advice of a doctor.

    Do not take the medicine if you are pregnant or a breastfeeding mother without consulting your doctor.

    Budecort® Rotacaps are contraindicated in Status Asthmaticus and an acute attack of asthma.
